"Always loaded"! A firearm should be considered as such as a sound safety practice.

The term is being conflated with sound/lawful practice..a totally different definition.

It is also worth noting that in Michigan, the penalty for carrying an unloaded gun is the same as if it were loaded.
Opinions and comments made by me on this forum should not be considered legal advice.

".......unloaded in barrel and magazine" was written by some non-gun genius and was speaking to an attached box magazine, not a detachable mag. In the case of the detachable mag, rifle, handgun or shotgun it must be IN the gun to consider the gun loaded.

Is such a gun loaded if a round is not in the chamber?? IMHO ---yes. If I have the mag in a shirt, or other pocket on my person-- then NO.. I am not a lawyer nor ever been on a jury.
