Trust: Multiple Form4s - Same Time?

Have to send a few form 4s in for three separate items. Can I mail them all together? If I can, can I....

-have one check for the full sum of the taxes? (or do I need one for each form4)
-have one copy of my trust? (or do I need one for each form4?)
-have one copy of my Cert of Compliance ie 5330-20? (or one for each form4)
