Don't forget that you can't carry concealed until you have it replaced.

Mike, IIRC it is the same form as for new /renewal CPL's I believe there is a check box for replacement card on it.

Nope, not on there. Check boxes for initial application, temporary, and renewal.

The only thing the law says is:
    MCL 28-425b
    (15) If an individual licensed under this act to carry a concealed pistol moves to a different county within this state, his or her license remains valid until it expires or is otherwise suspended or revoked under this act. A license to carry a concealed pistol that is lost, stolen, or defaced may be replaced by the issuing county clerk for a replacement fee of $10.00.
What ever you needed to get the first one, pictures etc, is what you would need to get a replacement. They should have a file with your last application in it, they will need to look that up for your CPL number and expiration date any way.

Actually I was just looking at it.

That form is only for new/renewal.

It's not for a replacement card.

When I had my passport pictures taken he took two of them. He told me that some counties required two photos.

When I went to get mine he didn't need the second picture but kept it. He told me if I lost my card to just come back and get a new one and don't tell the cops.
